Service Agreement
This Service Agreement is between RM Physiotherapy and the Participant/Nominated Representative in the National Disability Insurance Scheme.
This Service Agreement will be in effect from the NDIS plan start date for the duration of the Participant's association with RM Physiotherapy or until RM Physiotherapy is notified in writing by the Participant/Nominated Representative.
To engage the services of RM Physiotherapy, upon acceptance of this Service Agreement, the Participant/Nominated Representative will provide their NDIS plan details, NDIS Number and other relevant details as requested on this agreement, to RM Physiotherapy.
Services Provided:
The Provider agrees to deliver physiotherapy services to the Participant in accordance with the participant's NDIS plan. These services may include but are not limited to:

Assessment and evaluation of the participant's physical condition.

Development and implementation of individualized treatment plans.

Hands-on therapy, exercises, and interventions to improve mobility, strength, and function.

Education and guidance on managing physical health and well-being.
Responsibilities of the Provider:
The Provider agrees to:

Deliver physiotherapy services in a professional, competent, and ethical manner.

Develop treatment plans tailored to the participant's goals, needs, and preferences.

Keep accurate records of assessments, interventions, progress notes, and any other relevant information.

Regularly review and adjust treatment plans based on the participant's progress and changing needs.

Maintain confidentiality of the participant's personal information in accordance with privacy laws and regulations.

Provide timely communication and updates to the participant and their support network regarding progress, changes in treatment plans, and any other relevant matters.
Responsibilities of the Participant:
The Participant agrees to:

Actively participate in physiotherapy sessions and follow the recommendations and instructions provided by the physiotherapist.

Attend scheduled appointments punctually or provide advance notice if unable to attend.

Inform the physiotherapist of any changes in their health status, medications, or other relevant factors that may affect their treatment.

Provide honest feedback to the physiotherapist regarding the effectiveness of the treatment and any concerns or preferences they may have.

Be proactive in managing their overall health and well-being, including following any home exercise programs or self-care recommendations provided by the physiotherapist.
Fees and Payment:
The fees for physiotherapy services will be as specified in the participant's NDIS plan. The Provider will submit invoices for services rendered to the appropriate NDIS funding body in accordance with NDIS guidelines and requirements. The Participant is responsible for ensuring that their NDIS plan is adequately funded to cover the cost of physiotherapy services.
Cancellation Policy:
The Participant agrees to provide at least 48 hours hours' notice for any cancellations or rescheduling of appointments. Failure to provide timely notice may result in a cancellation fee being charged, as specified in the provider's cancellation policy.
Confidentiality and Privacy:
Both parties agree to maintain the confidentiality of all information exchanged during the provision of physiotherapy services. Personal information will be handled in accordance with applicable privacy laws and regulations.
Complaints and Dispute Resolution:
In the event of any concerns or complaints regarding the provision of physiotherapy services, both parties agree to first attempt to resolve the matter through open and constructive communication. If a satisfactory resolution cannot be reached, either party may escalate the matter through the appropriate channels as outlined in NDIS guidelines and procedures.
Term and Termination:
This agreement shall remain in effect until terminated by either party upon written notice to the other party. Termination of the agreement shall not affect any rights or obligations that have already accrued prior to the termination date.
